Laryngectomy is a surgical procedure in which the larynx (voice box) is partially or completely removed. This procedure is typically performed to treat advanced laryngeal cancer or other conditions that severely affect the function of the larynx. After a laryngectomy, the patient's ability to speak and breathe may be permanently altered. However, advancements in medical technology have provided various methods for patients to communicate and adapt to these changes.
Procedure
Anesthesia: The patient is given general anesthesia, ensuring they are completely unconscious and pain-free during the surgery.
Incision: An incision is made in the neck to access the larynx. The extent of the incision depends on whether a partial or total laryngectomy is being performed.
Larynx Removal: In a total laryngectomy, the entire larynx is removed. In a partial laryngectomy, only a portion of the larynx may be removed, preserving some vocal cord function.
Airway Reconstruction: After larynx removal, the surgeon creates a new way for the patient to breathe. This can involve creating a stoma (a small hole) in the front of the neck, known as a tracheostomy, which connects directly to the windpipe (trachea). This new opening becomes the primary airway.
Esophagus Reconfiguration: In cases of total laryngectomy, the surgeon may also reconfigure the esophagus to allow the patient to swallow and consume food and liquids.
Post-Procedure
Recovery and Healing: After the surgery, the patient is monitored closely in the hospital's intensive care unit. A tracheostomy tube may be in place temporarily to assist with breathing.
Voice Restoration: Since the vocal cords are removed, speaking in the traditional manner becomes impossible. Patients can learn to communicate using alternative methods, such as artificial voice devices, electrolarynx (a hand-held device that produces sound), or esophageal speech (using the esophagus to create speech).
Swallowing Rehabilitation: For total laryngectomy patients, swallowing therapy is essential to relearn safe swallowing and prevent aspiration.
Stoma Care: If a stoma is created, proper care and cleaning are necessary to prevent infection.
Cancer Treatment: Laryngectomy is a vital treatment option for advanced laryngeal cancer, potentially saving a patient's life.
Pain Relief: For patients with advanced cancer or severe laryngeal conditions, laryngectomy can provide relief from pain and discomfort.
